High School Admissions

A. Junior and Senior High School Level

For Incoming First Year Junior and Senior High School Students

1. Original copy of Report Card Form 138 (Report Card duly signed by the Principal)
2. Photocopy of birth certificate (PSA Copy)
3. Certification of good moral character from last school attended, signed by the Principal
4. Two (2) copies of 2×2 ID picture

For Transferees

1. Honorable dismissal from the last school attended
2. Original copy of form 138 signed by the principal
3. Certification of good moral character from last school attended, signed by the Principal or guidance counselor
4. Good academic grade (no failing grades during the previous curriculum year)
5. Interview by the Committee on Admission: Principal, Prefect of Discipline, Guidance Counselor
6. Written conformity with school rules and regulations attested by the parents/guardian
7. Two (2) copies of 2×2 ID picture
8. Physical presence of the transferee is required during enrollment

Note: All transferees shall be enrolled on a probationary status until all necessary credentials have been submitted to the Office of the Principal.

B. Foreign students

1. Permit to study and placement from the Department of Education
2. Photocopy of the Alien Certification of Registration (ACR)
3. Requirements listed under numbers 2 to 8 for transferees

NOTE: All the incoming students of Good Samaritan Colleges are required to take an Assessment Exam, before proceeding to the Enrollment Process. The Student Counseling Unit of SAO Department will be the one to administer the said Assessment Exam.
